Mercer County, Princeton, Greater Mercer Public Health Partnership Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP).
The Princeton Health Department (PHD) participated in a county-wide community health planning effort in 2012 and in 2015. PHD worked with community stakeholders to develop a Community Health Assessment (CHA) and a Community Health Improvement Plan (CHIP). Once our strategic planning process is complete (Winter 2015), our strategic plan, the CHA, and the CHIP will serve as prerequisites to fulfill requirements for National Public Health Accreditation and will be conducted every five years.
Community Health Assessment (CHA)
The CHA will describe the conditions affecting health, health status, and health needs of the population of Mercer County. Developing the CHA has been a collaborative process, including input from community stakeholders to determine the community health measures for the report.
Community Health Improvement Plan (CHIP)
The CHIP will serve as a strategic plan for improving health in Mercer County and Princeton, addressing the most important health problems and providing accountability via measurable, strategically planned goals. The initial round of community engagement marks the beginning of a long-term effort to strengthen PHDs relationships with stakeholders and help all partners move forward in a systematic way.

Performance / Dashboard Reports
Beginning in 2016, the Princeton Health Department (PHD) began creating strategic performance dashboards on a quarterly basis to monitor the status of key performance indicators (KPIs) to assist the department stay on top of goals and objectives. These goals and objectives come from PHDs 2015 through 2020 strategic plan, PHDs workforce development plan and the Greater Mercer Public Health Partnership (GMPHP) Community Health Improvement Plan.
